(Paper) Specimen collection form
We recommend a paper backup form is used in the field to record and track the use of barcodes for specimens, to support the coordination of samples and supervision. This template can be used by countries and adapted according to local requirements.
Consent forms
Consent in 'standard' surveys is typically collected verbally and recorded in the phone-based data collection forms. For surveys involving the collection of specimens, it is more likely that written consent will be a requirement by local ethics boards. The following forms are optional templates that can be adapted by countries as needed.

This generic template, if used, should be adapted per the survey protocol and local ethical requirements.
